Combigan and Pregnancy

If you are pregnant, your healthcare provider may recommend Combigan (brimonidine/timolol) if the benefits outweigh the risk. An increased risk of miscarriage was reported in studies where pregnant animals were given high doses of timolol (one of the active ingredients in the medication). Based on the results of these studies, the eye drops have been given a pregnancy Category C rating.

Is Combigan Safe During Pregnancy?

Combigan® (brimonidine/timolol) is a prescription eye medication approved for the treatment of high eye pressure and open-angle glaucoma. At this time, it is unclear if this product is safe for use during pregnancy.
 
It is important to understand that even though Combigan is an eye drop, some of the medication reaches the bloodstream and could, therefore, affect a developing fetus.
 

What Is Pregnancy Category C?

The U.S. Food and Drug Administration (FDA) uses a category system to classify the possible risks to a fetus when a specific medicine is taken during pregnancy. Pregnancy Category C is given to medicines that have not been studied in pregnant humans, but do appear to cause harm to the fetus in animal studies.
 
In addition, medicines that have not been studied in any pregnant women or animals are automatically given a pregnancy Category C rating.
 
Combigan was given a pregnancy Category C rating due to the timolol content. Giving very large doses of timolol to pregnant animals increased the risk of miscarriages. The other active ingredient in Combigan (brimonidine) did not cause any problems when given to pregnant animals, even in high doses.
 
It is important to note that animals do not always respond to medicines in the same way that humans do. Therefore, a pregnancy Category C medicine may be given to a pregnant woman if the healthcare provider believes that the benefits to the woman outweigh any possible risks to the unborn child.
